Employees select benefits during open enrollment
In addition to being a great place to work, the City of Carrollton offers a full complement of benefits to employees. Annual benefit plans become effective on January 1.
Insurance
Health CoverageThe City of Carrollton offers three medical plans to choose from: Advantage, Basic and HRA Plans. Summary information and rates for these plans are available in the 2011 Benefit Highlights. United Healthcare (UHC) is the preferred provider network. Always verify if your physician or facility is affiliated with UHC for maximum benefits. You can access a list of providers at TML IEBP.
Life InsuranceThe City offers Basic Life Insurance policies for eligible employees. An Accidental & Dismemberment Life Insurance Policy is also offered. If eligible, both policies are paid at two-times the annual salary. Supplemental Employee & Dependent Life Insurance can also be purchased. Long Term DisabilityCoverage provided by the City. Dental CoverageCarrollton offers two plans to choose from: High Dental and Low Dental Plans. Both plans allow you to select a dentist of your choice. Summary information and rates for these plans are available in the 2011 Benefit Highlights. Vision CoverageThe City offers a vision Plan with Superior Vision Services. Prescription BenefitA retail and mail order prescription drug program is available through REstat. Information regarding the drug program is available at www.tmliebp.org. After loggin in, click on the "Prescription Benefits" selection under "Quick Reference".
Employee Assistance Program (EAP)Employees have confidential access to the Employee Assistance Program. To access the Deer Oaks EAP log on to www.deeroaks.com To pre-certify all Deer Oaks benefits, please call 214-559-2171 or toll free at 877-616-8351. Retirement Plans Texas Municipal Employee contributes 7% - City matches 2:1.

Deferred CompensationThe City offers two 457 plans: ICMA and Nationwide. If an employee contributes 4.65% per paycheck, the City will match with 2.35%. Vesting is immediate. For more information on ICMA visit www.icmarc.org or for more information on Nationwide visit www.nrsretire.com
Roth IRAThe City offers an additional way to save for retirement through a Roth IRA. Contributions are made on an after-tax basis and can be payroll deducted.
RHSRetirement Health savings account offered through ICMA for eligible employees.
Social SecurityThe City does not participate in Social Security. If you have specific questions regarding your social security benefits please click www.ssa.gov
MedicareThe City contributes 1.45% to Medicare along with the employee contribution of 1.45%.
